When should I arrive for my appointment and what should I bring?:

Unless you are going to be sedated or asleep for a procedure, we ask that you please arrive 10 minutes prior to your scheduled appointment time.  If you are going to be sedated or asleep, we ask that you please arrive 15 minutes prior to your scheduled appointment time.  If you are coming for a consult appointment, we ask that you arrive 5 minutes prior to your scheduled appointment time.

Please bring the following with you:

  • a face covering, which is to be worn in our offices at all times, except when you are receiving treatment
  • a list of all medications you are currently taking
  • if you have not done so already, please go to the following link and complete your medical history form  https://forms.crescentoralsurgery.com/registration-form/new
  • any referral letters and/or x-rays given to you by your referring office;
  • your health card, or another form of government issued ID, in order to receive a prescription for pain medication; and
  • if you are to be sedated or asleep for your procedures, one responsible adult must accompany you to your appointment, wait while the procedure is being performed, and take you home.
  • if you are not to be sedated or asleep and you are over the age of 18, please come to your appointment alone.

Payment Information:

For your convenience, we have various methods of payment available for you to settle your account on the day of your appointment, including Cash, Debit, Visa, AMEX and Mastercard.

Unfortunately we are not able to directly bill your insurance, but our staff will help you with the appropriate insurance forms for submission to your carrier.

Cancellation Policy:

If you are unable to attend your appointment we ask that you provide a minimum of 72 hours’ notice to avoid a cancellation fee.

Medication:

If your surgeon has asked you to begin medication prior to your surgery please ensure to do so.  Unless instructed otherwise, you should also continue to take all your regular medications at the appropriate time.  If you are to be sedated or asleep for your procedure you should take your medications with a sip of water and not with food.
