PROCEDURES WHERE THE METHOD OF SEDATION IS NOT PREDETERMINED

Please ensure that when you confirm and come to your appointment, you still meet the screening criteria for COVID-19.  The questions can be found on our website at https://www.crescentoralsurgery.com/wp-content/uploads/2021/09/Covid-19-Screening-Website-Document-8Sept2021.pdf.

Please arrive 10 minutes prior to your appointment time and if this is your first appointment, please bring a list of all medications you are taking and any referral letters and/or x-rays given to you by your referring office. You must wear a face covering to our office.

If you plan to be asleep for your procedure, have one responsible adult accompany you to your procedure and do not have anything to eat or drink for 8 hours before your surgery.   If you plan to only have dental freezing please come alone, unless you are under the age of 18 and then you can bring one guardian with you.  We require Government Issue ID, preferably your health card, for you to receive a prescription for pain medication.

If your surgeon has asked you to begin medication prior to your surgery please ensure to do so.  Unless instructed otherwise, you should also continue to take all your regular medications at the appropriate time, but only with a sip of water and not with food, if you plan to be asleep.

We have various methods of payment available for you to settle your account in full and our staff will assist you with your insurance forms.
